Download: Financial Aid Form 2007/2008 (PDF)

Limited Financial Aid is available on a sliding scale based upon income. Financial Aid is awarded to families who would otherwise not be able to afford to enroll. Applications for Financial Aid are available from the Opus 118 office and must be returned with the most recent copy of a Federal Tax Return to verify family income level or proof of household income. Financial Aid application forms are required to be submitted each year.  Financial Aid Awards are valid for Fall/Winter 2007 Term, Winter/Spring 2008 Term and Summer 2008 Term. Maintenance of the Financial Aid Award is contingent on regular attendance, consistent progress and the willingness of both student and parent to adhere to the following:

  • Students must attend each scheduled lesson.
(No more than two unexcused lessons per semester.)

  • Students must arrive on time for lessons.
(Students arriving late more than three times may lose their Financial Aid Award.)

  • Students must bring their instruments to each lesson.
(Opus 118 will not supply instruments for lessons.)

  • Tuition payments must be up to date and payments must be made on time.
(Accounts two months in arrears will constitute a forfeiture of a Financial Aid Award.)

  • Any termination of awards will be preceded by a written notice and explanation of the problem identified.
